We discuss which activities may need help, what support is already in place and whether nursing services have been recommended. With the appropriate permission, we review relevant discharge information and clarify Abloom's role alongside your existing providers.
Help with daily routines
Depending on the assessment and the accepted plan, support may include personal care, meals, light household tasks and agreed mobility assistance. Nursing services are reviewed separately according to your clinical needs.
Know who handles each part of care
You may have a primary clinician, a therapy provider, a pharmacy and other people involved. We clarify which tasks belong to Abloom and which remain with those providers. If instructions or support needs change, contact the appropriate care professional so the plan can be reviewed.

- Can you begin on the day of discharge?
- Start dates depend on the assessment, any required orders, staffing and the completion of service arrangements. Confirm the date with Abloom before relying on coverage.
- Do you provide physical or occupational therapy?
- Ask which services Abloom directly provides. We can discuss coordination with an existing therapy provider; therapy should not be assumed to be part of an ordinary caregiver visit.
- What should I bring to the first discussion?
- Start with the anticipated return-home date, the care location and a general description of the support recommended. We will explain how to share relevant records securely when that is needed.
