How to make a complaint

If something about your care is wrong, tell us. You can raise a concern about anything: the care itself, a caregiver or nurse, a visit that did not happen, a bill, a missing belonging, or the way somebody spoke to you.

This page applies to everyone Abloom cares for, whether that is at our residence or in your own home. You can complain yourself, or somebody can do it for you: a family member, a legal representative, a guardian, or anyone you ask to speak for you.

Who to tell

Complaints go to Abloom’s Administrator. There are three ways to reach them, and they all arrive in the same place.

  • By phone. Call (612) 409-4351 and say you want to make a complaint. Ask for the Administrator.
  • By email. Write to care@abloomseniorliving.com. Put "complaint" in the subject line so it is not read as a general enquiry.
  • In writing, through this website. Use the complaint form on this site. It reaches the same person, and it gives you a written record of what you said.

What happens next

We receive it

However you raise it, your complaint goes to Abloom's Administrator. You do not have to put it in writing, and you do not have to use a particular form of words.

You can raise a complaint yourself, or someone can raise it for you: a family member, a legal representative, a guardian, or anyone you ask to speak for you.

We look into it

The Administrator investigates. That means finding out what happened: reading the record of the visit or the day, speaking to the staff involved, and speaking to you about what you saw or were told.

If your complaint is about a particular caregiver or nurse, you can say so, and you can ask that they not be sent to you while it is being looked into.

We tell you what we found

The Administrator comes back to you with what was found and what is being done about it. If we got something wrong, we say so.

If you are not satisfied with the answer, tell us, and you can take it to the state at any point. You do not have to come to us first, and you do not have to wait for us to finish.

We keep a record of it

Every complaint is written down: who made it, what it was about, what we did to look into it, and how it ended. Minnesota requires us to keep that record for at least 2 years and to show it to the Minnesota Department of Health if they ask (Minn. Stat. 144A.4791, subd. 11).

Making a complaint will not be held against you

Abloom does not discriminate or retaliate against anyone for making a complaint or raising a concern. This is a promise we are required to make and one we intend to keep.

  • Your care will not be reduced, delayed or stopped because you complained.
  • Your services will not be terminated because you complained.
  • Nobody will be treated differently in their own home for having raised something.
  • It makes no difference whether you complain to us or straight to the state.

If you think something has changed about your care because you raised a concern, that is itself something to tell us, and something the state will look into.

You can go to the State of Minnesota instead

You do not need our permission and you do not need to come to us first. These offices are independent of Abloom, and you can contact any of them at any point, whether or not you have told us anything.

Minnesota Department of Health, Office of Health Facility Complaints

The state office that licenses Abloom and investigates complaints about licensed home care providers. This is the regulator.

Minnesota Adult Abuse Reporting Center

For suspected abuse, neglect or financial exploitation of a vulnerable adult. Answered 24 hours a day, every day. If someone is in immediate danger, call 911 first.

Office of Ombudsman for Long-Term Care

Free, independent advocates who are not part of Abloom and not part of the regulator. They will talk through a concern with you, and help you raise it, whether or not you want to make a formal complaint.

Our record of it

Minnesota requires us to write down every complaint, what we did about it and how it ended, to keep that record for at least 2 years, and to show it to the Minnesota Department of Health if they ask for it. This page, and what it promises, is our written complaint procedure under Minn. Stat. 144A.4791, subd. 11.
