Abloom's family portal for in-home care.

When you are involved in someone's care, a brief note saying everything is fine may leave you with questions. Abloom's family portal gives authorized users a place to review shared care information and to understand the follow-up on documented concerns.

The portal is for people authorized to receive information about the client's care. What each person can view follows the permissions and sharing arrangements agreed for that care.

Care information you can understand

Review the care updates made available to you, including completed support and relevant observations. The goal is to help you understand the day-to-day care without having to interpret unfamiliar clinical language.

Changes in the context of a familiar routine

A person's usual pattern gives useful context for a care conversation. Where an item is part of the plan and has been recorded, the portal can present changes over time, such as changes in meal intake or in the amount of assistance needed.

These records reflect documented observations. They are not continuous monitoring, a diagnosis, or a guarantee that every change will be detected.

Who reviewed it, and what happened next

When a concern is documented and shared, you can see the recorded review and follow-up: the person or role that reviewed it, the action taken and any next step. Questions that require nursing judgment are directed to the appropriate clinical professional.

A place for routine questions

Use the available messaging features for nonurgent questions. At the start of care we explain when messages are reviewed and which phone contact to use for time-sensitive concerns. For an emergency, call 911.

Does the portal continuously monitor health?

No. The portal displays information that has been recorded and shared. It does not observe a person between visits and it does not replace a clinician's assessment. For time-sensitive concerns, use the care contact provided when service begins.
